If you are a school or private club and need to schedule ImPACT testing for your team, please enter your contact information below. Under the comment section please let us know that you are a Team Administrator and need to schedule testing for your team. We will contact you in order to setup a time that will accommodate your entire team. If you are an individual who is enrolling in a concussion plan by yourself, please enter all of your information below. If you would like us to contact your private club team or school and make them aware that you are taking the pro-active measure to enroll yourself in a concussion plan please enter their contact information in the comment section and we will contact them for you.
